If you have a medical condition that can be alleviated through the use of medical marijuana, obtaining your medical marijuana card is very straightforward. Below is a summary of the application process.
Colorado Medical Marijuana Card Registration Requirements:
• Complete an application form for a medical marijuana registry (ID) or identification card
• Sign, date and have the the registry application form notarized
• Have the physician sign and date the physician recommendation form. The doctor visit fee is $60.
• Photocopy a legible copy of an approved photo I.D. that establishes Colorado residency.
(bring Colorado ID or current ID with a copy of an Excel, water or recent phone bill)
• Send in a non refundable application fee of $35 (check or money order payable to (CDPHE))
• Send all of the items above to:
Colorado Department of Public Health and Environment or (CDPHE)
in care of the Colorado Medical Marijuana Registry
